YES! I Make It Happen Employee Recognition Program

In order to recognize faculty, staff or student workers, please complete each page of this online form. A description of the faculty, staff or student worker's specific efforts should be indicated in the description field provided on the final page. Upon completion of these online forms, an email notification will be sent to the person(s) being recognized, Employee Relations, and to the supervisor of the faculty, staff or student worker being recognized. Note: You are able to select multiple names for the same nomination, if applicable. All full-time and part-time faculty, full-time and part-time staff, and active student workers are eligible for this program, and will be considered for a monthly and quarterly reward as well as an annual recognition award. All submissions must be completed by the last day of each month in order for the nominee to be considered for a reward that month. The winner of the random monthly drawing will be acknowledged at the beginning of the following month.
